The Story Behind The Technology

Most AI documentation companies were built by technologists who learned medicine as they went, essentially working backwards toward something that read like a physician wrote it. iScribe went the other direction. Here’s how we got there.

Season 12015

A Better Way to Document

Before founding iScribe, Pat Williams and Justin Hipps worked with various health-tech startups and watched firsthand as physicians struggled with clunky EHR templates and awkward microphones bolted to desktop computers. These sub-par tools made documentation slow and frustrating. So they developed an EHR-integrated mobile app with built-in speech-to-text, a response to the growing popularity and importance of the iPhone and iPad.

EHR integration set the new company apart immediately. Beyond voice documentation, providers could now select diagnoses, place orders, and complete E&M coding—all from a mobile device that allowed them to document naturally anywhere. For the first time, clinicians could complete the entire encounter without being tethered to a desktop computer or bogged down by the complexity of their EHR.

Season 22016–2022

The Rise of the Human Scribe

Speech-to-text kept providers mobile, but our busiest customers wanted more. They didn’t want to dictate section by section, they wanted to talk through the visit and have someone else write the note. So we built Virtual Scribe Service (VSS). Providers recorded the full encounter using the same app developed in Season 1 on their iPhone, and a remote scribe turned the audio file into a finished note. When COVID-19 restricted who could be in an exam room, VSS became essential. Practices that could no longer host an in-person scribe (still the norm across much of the industry) kept documenting without missing a beat.

Deepening Our Impact

We expanded our EHR integrations, and VSS grew to more than 150 scribes working remotely across the country. But a national scribe workforce came with real friction: coverage, time off, and the frequent turnover often associated with the role. Season 2 was pivotal in that it hard-coded physician documentation quality and established our world class customer support—two things integral to our DNA.

Season 32023–2024

The AI Transition

Our VP of Product, Mike Cardwell, saw how impactful AI could be in the hands of our customers. We’d already spent nearly a decade accumulating something no algorithm could ever replicate: the institutional knowledge of scribes who understood exactly what great documentation looked like. So we began building iScribe AI, training it on the standards our own data set had created. Early adopter physicians told us plainly when a version wasn’t up to their standards, and our long-tenured scribes engineered the prompts until our engine performed to their expectations. The result? A fully automated documentation experience and a customer base that transitioned from traditional scribes to AI without losing a single provider. For the first time, providers were truly free to be fully present with their patients.

Season 42025 & Onward

New Intelligence

We launched iScribe Code Intelligence in beta, our first step towards helping providers mitigate documentation-related denials. This powerful product utilizes AI to recommend an E&M code with 95% audited accuracy supported by in-house Certified Professional Coders and detailed analysis of AMA MDM guidelines.

Continuing the Journey

We expanded iScribe AI and iScribe Code Intelligence for urology with more specialties on the way. In 2026, we introduced iScribe Flex, a lightweight way for independent providers to get started. The next chapter is already underway with the development of real-time denial prevention tools.
